Breakthrough is a DHS and ADOT-approved facility offering DUI screenings, education, and treatment. We also provide the "revocation packet" for those attempting to get their license reinstated.

To make your appointment as smooth as possible, please bring the following items:

• Your Arizona Driver’s License or ID
• The ticket/violation notice with the exact date of your DUI violation
• The name of the court handling your case and any associated paperwork
• Any certificate if you have already taken a DUI Education class
• A copy of your Extended Motor Vehicle Report (MVR), a 5-year record certified by the AZ Department of Transportation, which costs $5 and can be ordered online via AZ MVD Now
• If applying for license reinstatement, bring the filled-out Revocation Application packet
