As a Patient-Centered Medical Home, SLWIC employs an innovative, interdisciplinary team-based approach to healthcare, providing access to evidence-based care, education, and self-management support. SLWIC offers a wide range of primary healthcare services, including pediatric care, obstetrics and gynecology, family planning, internal medicine, and specialized services for women, teens, and men. We also provide laboratory services, diabetes management, weight loss control, asthma and COPD treatment, immunizations, and more. Our integrated and comprehensive model ensures a continuum of primary and behavioral healthcare for both children and adults.

Our behavioral health services include substance abuse evaluation and treatment, buprenorphine/naloxone maintenance for opioid dependence, family and individual therapy (including anger management), psychiatric health and medication management, and family support services such as case management and peer support. We also offer DUI screening, treatment, and education services, misdemeanor domestic violence services, and group support counseling sessions.

To enroll in our services, clients need to bring a photo ID, Social Security card, current insurance card, and any necessary referrals or court/probation paperwork. During intake, we assess the client's financial situation to identify potential funding sources.
