DUI Evaluation (Uniform Report)
- Purpose: Required for court after a DUI arrest.
- Scheduling: Same-day or next-day appointments are often available.
- Documents Needed:
- Driving Abstract: Obtainable from the Driver’s License facility (must be no more than 30 days old).
- Arrest Documentation: Includes date/time of arrest and BAC or refusal details. Found on documents like the “Notice of Summary Suspension” or “Law Enforcement Sworn Report.”
- Note: Evaluations cannot be completed without these documents.
Assessments
- Purpose: Determine the extent of alcohol or drug problems and the need for treatment.
- Methods: Utilizes diagnostic instruments approved by SUPR (Illinois Department of Human Services Division of Substance Use Prevention & Recovery) and SOS (Secretary of State).
- Treatment Placement: Required for entry into Level I or Level II treatment programs.
