Among these services are IPV Case Management and Intervention Services, which provide support and resources for individuals affected by interpersonal violence. We also offer Holistic High Intensity Enhanced Case Management and Medical Respite Care, ensuring that clients receive tailored assistance that promotes their overall well-being. For those transitioning from hospitalization, we provide Short-Term Post Hospitalization Housing to support their recovery journey. Our transportation and home visiting services further enhance access to care, while the Healthy Opportunities Pilot aims to address social determinants of health.
